INDICATIONS

It is used in the treatment of catabolic or tissue-reducing processes. Oxandrolone is indicated in conditions such as chronic infections, major surgery, (myopathy induced by corticosteroids, pressure ulcers, burns), or severe trauma, where it is required to stop the effects of catabolic processes or loss of proteins.

It is important to remember that these types of substances are adjuvants in these cases and do not replace the conventional treatment of these disorders. It is also used to treat Turner syndrome, to treat the short stature seen in these patients. However, the theraphy is still controversial, recent experiment reports show that oxandrolone can be effective as a growth hormone and that it can increase the effectiveness of growth hormone therapy.

MECHANISM OF ACTION

It reverses the catabolic processes and the negative nitrogen balance improves protein synthesis and stimulates the appetite if there is a simultaneous intake of calories and protein.

DOSAGE

In adults, 2-4 weeks of therapy is generally recommended.

Doses in adults and adolescents, 2.5 mg twice or four times a day.

The maximum dose is 20 mg per day.

Doses in children are 0.25 mcg (0.25 mg / weight / day).

In cases of Turner syndrome, the doses are 0.05 to 0.125 mg / weight / day. Patients should start generally and maintain a minimum effective dose to minimize adverse effects.

CONTRAINDICATIONS

Anabolic steroids are commonly used to treat symptoms associated with osteoporosis. However, long-term use is not recommended due to the questionable effects of these agents and the risk of serious side effects does not warrant use.

The use of anabolic steroids is not recommended in athletes.

ADVERSE EFFECTS

Virilization, acne, clitoral enlargement, hoarseness, menstrual irregularities, hair loss have been observed in women.

In prepubertal men, acne, penis enlargement, increased frequency of erections, abnormal hair growth has been observed.

In men after puberty, bladder irritability (frequent urge to urinate), chest pain, breast enlargement, frequent erections have been observed. Less frequently, anemia, iron deficiency, loss of appetite, edema, nausea, vomiting, unusual bleeding have been observed.

Hypercalcemia (depression, unusual tiredness), unusual darkening of the skin.

In elderly patients, carcinoma of the prostate or hypertrophy of the prostate (difficulty or frequent need to urinate) can be seen.

In rare cases of liver necrosis (black stools, headache, vomiting blood, bad breath), liver cancer (stomach ache or abdominal pain, unexplained weight loss), peliosis hepatis (dark urine, loss of appetite, red spots on skin, mouth, nose, throat, fever, vomiting) nausea has been observed.

Interactions with other medications

The use of anabolic steroids with adenocorticosteroids, glycocorticosteroids, drugs or foods that contain sodium can increase the possibility of edema.

Concomitant use of oxandrolone with anticoagulants may increase the effect of the anticoagulant.

Anabolic steroids can lower glucose levels, diabetic patients should be monitored for probable symptoms of hypoglycemia, and doses of hypoglycemic agents or insulin should be adjusted.

Concomitant use of anabolic steroids with hepatotoxic drugs can increase hepatotoxicity. Patients with liver disorders should be carefully monitored.

PRECAUTIONS AND WARNINGS

There are no in vitro or animal mutagenicity studies with oxandrolone.

Animal studies receiving 9 times the human dose have shown that oxandrolone can cause embryotoxicity, fetotoxicity, and infertility and increased masculinization. Therefore, it should not be used during pregnancy.

It is not known whether it is excreted in human milk, but its use should be avoided while the woman is breastfeeding.

Use with caution in children and adolescents due to early closure of the epiphysis. Medications should be withdrawn if there are no results after 6 months of treatment

OVERDOSE

Treatment of overdose is symptomatic and supportive. In case of overdose, treat the patient with emesis and gastric lavage. Liver function should be monitored, as hepatotoxicity is a serious adverse effect.
